CareBridge is a connected digital healthcare platform designed to solve one of healthcare’s persistent challenges: fragmented patient information and disconnected healthcare services.

Built during a hackathon, CareBridge connects patients, hospitals, clinics, laboratories, pharmacies, and insurance providers through a unified digital healthcare experience. Patients can access their medical history, laboratory results, prescriptions, and appointments in one place while retaining control over which healthcare providers can access their records. Healthcare providers can manage patient information, issue prescriptions, coordinate appointments, and access relevant medical history with permission.

Rather than treating each hospital visit, laboratory test, or prescription as an isolated interaction, CareBridge aims to create a more continuous and coordinated healthcare journey. By reducing information fragmentation and improving how healthcare stakeholders connect, we aim to make healthcare easier to navigate for patients and more efficient for providers.

Our vision is to help build a future where patients remain at the centre of their healthcare, medical information moves with appropriate permission, and healthcare services work together rather than in isolation.
